Summary

Mitral valvo roplacement (MVR) is still associatod with a relatively high mortality. To proovo the benofits of chordal preservation at mitral valvo replacement, we investigated its effects in a series of 65 consecutive MVR patients. Of those patients, in 42 preservation of the mitral subvalvular structures was possible whoreas in the other 23 they had to be excised. Both groups showed no differences in age, sex, preoperativo NYHA class, and valve pathology. Intra- and postoperative manage ment was similar in both groups. The surgical techniques employed are described and the early postoperative course of both groups are analysed. Clinical, electrocardiographic and echocardiographic investigations, measuring left-atrial and -ventricular diameters, right-ventricular diameters and left ventricular length, domonstrated that whereas beneficial effects were evident in both groups, the amount of benofits was higher in patients with chordal preservation. Chordal preservation also provided less arrhythmias than chordal resection.

Zusammenfassung

Der Ersatz der Mitralklappe mit Klappenprothesen ist immer noch mit einer relativ hohen Mortalität behaftet. Um den Einfluß der Erhaltung des subvalvulären Apparates der Mitralklappe bei dem Ersatz der Mitralklappe in der frühpostoperativen Phase zu bestimmen, wurde in unserer Abteilung eine Serie von 65 konsekutiven Patienten untersucht. In dieser Patientenserie wurde in 23 Fällen der subvalvuläre Apparat reseziert und in 42 Fällen erhalten. Beide Gruppen zeigten keine signifikanten Unterschiede in Alter, Geschlecht und präoperativer NYHA-Klassifikation. Das intra- und postoperativo Management war in beiden Gruppen gleich. Klinische, elektrokardiographische und echokardiographische Untersuchungen, bei denen linksatriale, linksventrikuläre und rechtsvontrikuläre Durchmesser sowie die linksventrikuläre Länge gemessen wurden, zeigten, daß beide Patientengruppen von der Operation profitierten. Wurde der subvalvuläre Mitralklappenapparat erhalten, profitierten die Patienten deutlich mehr. Außerdem war die Erhaltung des subvalvulären Apparates mit einer geringeren Häufigkeit des Auftretens von Arrhythmien verbunden.
